The first Case Manager among HMC facilities (In-patient and Community)

Enormous experience in healthcare, including extensive experience in the community healthcare setting, which is rare in Qatar. 
An original team member who established the Home Healthcare Service (HHCS). HHCS was one of the first community services in the Middle East to be JCIA accredited and subsequently received reaccreditation.

My role as Director of Nursing in the HHCS, prepared me to formalize a structure for the Private Nursing Services when I was appointed as AEDON in 2014.  

Developed the process to support nurses in the community (in patient’s home).

Initiated the process of safety discharge for long term patients from HMC facilities to patient’s home. 

In collaboration with Outsourcing Agencies, returned 300 HMC staff in the hospital and replace with Outsourcing Agencies Staff to provide nursing provision in the community. 

Increase  Capability, and  productivity of Staff Nurses by reducing their overtime.

Currently, working on Planetree Person-Centered Care Certification in the community ( Private Nursing Services  and Home Healthcare Services are among the Five Facilities in HMC who were been selected to obtain Person Centered Care Certification (PCC), initiating partnership between patients, patient’s family, staff and leaders in delivery compassionate care in the community).
Established PNS agency committee in partnership with agencies to monitor the quality of patient care ensured that standardized of the provision of patient’s care (e.g. policies, procedures and protocols). 

Private Nursing Services was the first department in HMC who opened Quarantine Facilities on the 27th February 2020. When we started this journey, no one had an idea how to handle suspected Covid-19 cases but, we created systems that streamline the operational activities, and this empowered the staff nurses on how to deal with day to day challenges.
