The aim of Complex Care Task Force is to implement and monitor the transformation of care through the establishment of an innovative, effective and efficient model for serving medically fragile children and their families within HMC.

The Complex Care Clinic aims to improve quality of life for children and young people with complex medical conditions and their families, Improve their health status, wherever possible, maximize time out of hospital, decrease avoidable hospitalizations, days in hospital, inefficient, unnecessary or avoidable ambulatory clinic visits, and emergency department visits.

To achieve all above goals, the Complex Care Task force was established with an inter-professional team by hiring FTE’s in physiotherapy, dietetics, respiratory therapy, nursing and speech therapy. SickKids SMEs presented three inter-professional workshops for the Complex Care Team. Main Highlight of 2012 is opening of the Complex Care Clinic on 2 October 2012 at Hamad General Hospital. A total 27 patients have been assessed in the clinic so far. It was identified that there is need for additional clinics to ensure continuity of care for complex patients who require allied health therapy sessions. To this end, the Complex Care Team established a new Therapy Clinic. The focus for 2013 is to start Complex Care Inpatient Services and work with the Home Care Team to ensure community support is available for families.
