Health Literacy, Learning Style, and Language Assessment


To ensure that every patient can actively engage in their care, we assess each individual's ability to understand health information, their preferred way of learning, and any language needs. This allows us to tailor our communication and education approach, making complex information easier to understand and retain.

We use a variety of evidence-based methods to support patient understanding and participation, including:

  • Teach-Back – Patients are asked to repeat information in their own words to confirm understanding.
  • Show Me – Patients demonstrate how to carry out care instructions or use medical devices.
  • Chunks and Checks – Information is provided in small sections with regular pauses to check for comprehension and invite questions. 

Social Needs Assessment

The Social Needs Assessment identifies social and economic factors that may impact a patient’s health and ability to manage their care effectively. It includes evaluating issues such as housing, employment, transportation, food security, and family support. By understanding these non-medical influences, we can connect patients with appropriate community resources and support services to help overcome barriers, enhance well-being, and promote continuity of care.

Advance Care Planning

Advance Care Planning empowers patients to express their wishes and preferences for future healthcare decisions in the event they become unable to communicate. This proactive approach ensures that care aligns with their values and goals, providing peace of mind for patients and their families while guiding healthcare teams in delivering respectful and personalized care.

Shared Decision-Making

Care Partner Program


The Care Partner Program allows patients to designate a trusted friend or family member to serve as an active member of their care team. This individual is involved in care planning, education, and decision-making, providing consistent support throughout the patient's healthcare journey, both during hospitalization and after discharge. The care partner helps ensure continuity of care, reinforces medical instructions, and offers emotional support, enhancing patient confidence, safety, and recovery.​

Whiteboard

A visual communication tool placed in each patient room to display key information such as care team names, daily goals, patient preferences, and important updates. It helps ensure everyone, including patients, families, and staff, stays informed and aligned on the care plan.

Bedside Shift Report

Bedside Shift Reports are conducted at the patient’s bedside, ensuring that patients and their families are actively involved in the exchange of care information. This transparent practice promotes collaboration, enhances safety, and gives patients an opportunity to share input or raise concerns in real time.​



Real-time Access to Medical Records

Patients are empowered to take an active role in their care by accessing their medical records and treatment plans in real time, under the guidance of their healthcare team. This transparency supports informed decision-making and helps patients stay engaged in their care journey. Records can be securely viewed online through the MyHealth Pat​ient Portal, accessible anytime and from anywhere.
