Program: Adult Day Care Rehabilitation Program
The Day Care Rehabilitation program is a comprehensive, flexible program which will meet the individual rehabilitation needs of the patient. Patients who are admitted into the program no longer require continuous medical and nursing supervision, enabling them to return home at night. Day Care Rehabilitation helps patients transition back to their home and the community.
Timings: 8:00 am - 6:00 pm / Saturday - Thursday
Program Description:
Day Care Rehabilitation is an individualized program which is medically prescribed and includes individual and group therapies.
Day Care Rehabilitation patients may receive the following:
- Physical therapy
- Speech therapy
- Occupational therapy
- Nursing services
- Recreational therapy
- Psychological counseling
- Case management
- Patient/family teaching
- Pre-vocational services
- Cognitive Retraining Program
- Assistive Technology Program
- ADL Retraining Program
A patient may be scheduled for some of the following groups, depending on individual treatment goals.
Group therapies available include:
- Community living skills
- Daily living skills
- Sensorimotor groups
- Social Skills
- Support/Coping group
- Recreation Group
- Home exercise
A lunch time meal is provided to all patients enrolled in the program.
Length of Program: The Day Care Rehabilitation program can range from one to eight weeks with individual goals dictating the length of the program, which can be extended as recommended by the rehabilitation team.
