Talk on reactive risk management i.e identification of risks via nonconformities, audits, complaints/user feedback.

To include RCA, investigation, corrective action, effectiveness check, RCA trending, , other risks identified via this process (i.e extent and impact)

Breakout session - we will give the delegates a scenario where something has gone wrong, and ask them to consider what actions they would take to resolve the immediate problem and reduce/remove risk of recurrence and other associated risks.
