HMC’s Transformative Journey Towards Person-Centered Care


What is Person-Centered Care (PCC)?

Planetree International defines Person-Centered Care (PCC) as “care focused on the needs of individuals, guided by peoples’ preferences and values, and includes supporting structures, policies, and practices that create a culture of quality, compassion, and partnership across the continuum of care.”

The philosophy of person-centered care focuses on:

  • Respect for patients’ values, preferences, and needs.
  • Coordination of care for more efficiency.
  • Comfort and emotional support for mental health.
  • Information, communication, and education.
  • Involvement of family and friends.

The Planetree approach emphasizes the quality of human interactions and caring communication, and practical strategies for engaging patients, families, and communities as partners in care.

The Person-Centered Care Framework​


To support the overarching vision of HMC, we have adapted the Person-Centered Care framework in our approach to care, which focuses on the needs of individuals, guided by people's preferences and values, including supporting structures, policies, and practices that create a culture of quality, compassion, and partnership across the continuum of care.

The PCC framework directly supports HMC’s goal of obtaining Certification for Excellence in Person-Centered Care across all its facilities. Using the PCC framework allows HMC facilities to align their operations and approach to care by implementing programs and policies guided by the following PCC primary drivers:
