• 0700H-1500H Sundays- Thursdays

Services Offered:

A. POST-NATAL SERVICES

Assessment for the following mothers who undergone the following in HMC facilities the past 2-14 days but not limited to:

1. Normal Deliveries recommended by the physicians
2. Vaginal Deliveries with 3^rd and 4^th degree tears
3. Post cesarean Section (Can be extended until 20 days)

Aa.Patient services delivered for the mother:

1. Physical Assessment and Reassessment

• Systemic examination (including wound care if applicable)
• Pain assessment
• Environmental safety
• Fall Risk Assessment
• Mental Well-being/baby blues
• Psychosocial assessment
• Breast feeding assessment

2. Patient- Family Education

B. NEONATAL SERVICES

Assessment of Neonates delivered across HMC facilities except high risk newborn (48 hours old until 14^th day old)

Ba. Patient services delivered for the baby:

1. Physical Assessment and reassessment including Transcutaneous Bili check and weight checking.
2. Patient- Family education
   • Information on general newborn care
   • Provide support on management of successful and exclusive breastfeeding.
