The Person-Centered Care Forum (PCCF) is a platform dedicated to fostering dialogue, collaboration, and innovation in healthcare delivery. It brings together patients, families, healthcare professionals, and stakeholders to share experiences, insights, and best practices in person-centered care. The forum aims to enhance the quality of care by promoting a culture that prioritizes the holistic wellbeing of patients and caregivers, ensuring that their preferences, needs, and values are at the forefront of care decisions. Through interactive sessions, workshops, and discussions, the PCCF facilitates the exchange of ideas and strategies to continuously improve patient experiences and health outcomes.
PCCF has been instrumental in advancing HMC's journey towards a more person-centered care approach and a real partnership with patients. It provides a platform for discussing a more empathetic and collaborative approach to care and highlights the importance of involving patients, their families, and caregivers in every decision impacting their care. In 2023, the Forum attracted 2,435 participants through virtual and face-to-face sessions, reflecting its significant impact and reach.

The focus of the annual forum is to bring together healthcare professionals who share a passion for creating patient and family experience that improves health outcomes and healthcare culture. The conference had three tracks this year: Innovation in Person-Centered Care Practice, Compassion – Empathy in Action, and Partnership and Co-designing Care. Read more

More than 3,500 healthcare professionals attended the Person-Centered Care Virtual Forum 2021. Formerly called the Patient Experience Forum, this unique event is being delivered by HMC in partnership with internationally renowned US-based healthcare consultants Planetree International.
The theme for the 2021 event, held virtually for the first time, was Compassion in Action: Spread Kindness and there were five main tracks: Measuring and Improving What Matters; Service Excellence; Driving a Transformation; Partnerships with Patients, Families, Staff, and the Community; and Leading in Person-Centered Care.
