Through the Patient and Family Advisory Council, we foster open collaboration among patients, family members, and our clinical and administrative staff. This ongoing partnership continuously enhances services and shapes a healthcare system that genuinely places patients at the center of their care journey. PFAC members contribute valuable perspectives and suggestions, helping us to refine our services to better meet patient needs. In our pursuit of Person-Centered Care, Home Health Care Services (HHCS) has adapted hospital standards for community settings. Engaging stakeholders—healthcare providers, patients, and families—enabled us to tailor these standards to our community's specific needs. Our goal is to enhance care quality, enhance health outcomes, and ensure a positive healthcare experience by treating each person uniquely, respecting their background, culture, beliefs, and personal circumstances. Since achieving Gold Certification for Excellence in Person-Centered Care in 2020, we've seen enhanced quality of care, increased patient satisfaction, improved clinical outcomes, better staff morale and collaboration among stakeholders.
The journey to person-centered care certification represents a transformative endeavor in delivering compassionate, comprehensive, and personalized care. Embracing this approach fosters a culture of empathy, cooperation, and respect that empowers individuals and enriches the overall healthcare journey. In 2020, Private Nursing Services earned Gold Certification for Excellence in Person-Centered Care following a rigorous evaluation by Planetree International. In 2024, we received approval from the Planetree International Review Panel to apply for Distinction Certification, which requires maintaining Gold-level standards while furthering person-centered care. To achieve this distinction, we have identified scalable practices for incorporating patient, resident, and family perspectives into ongoing healthcare improvements beyond certification requirements. One of our initiatives included our Patient and Family Advisors co-designing modules for the Compassionate Human Interactions Training, tailored for Arabic-speaking patients and families. Additionally, we launched other initiatives to reduce health disparities among vulnerable populations, including those with dementia.

Achieving the Gold Certification for Excellence in Person-Centered Care has transformed the Communicable Disease Center (CDC) into a supportive family environment. This approach extends beyond patient care, enhancing the wellbeing of everyone at CDC. A culture of listening and mutual support, especially during challenging times, strengthens bonds and fosters resilience among staff. This camaraderie enhances teamwork and creates a positive work environment where everyone feels valued.
Building on the success and principles established at CDC, Hamad General Hospital Emergency Department (HGH-ED) has also embraced the person-centered care approach. The lessons in creating a supportive, family-like atmosphere and the emphasis on mutual support and resilience have been instrumental in shaping our practices. By integrating these values, we ensure that the high-stress environment of emergency care is still grounded in compassion and empathy, fostering a cohesive and dedicated team.
